We are excited to have you join us at Temple Adas Shalom's Annual Purim Carnival. This form is not required as tickets will also be on sale during the event but is for anyone interested in pre-ordering tickets to skip the lines upon arrival. Tickets will be used for all purchases during the event, including but not limited to the following:
  • Games (2 tickets each)
  • Activities (1-5 tickets each)
  • Food (1-5 tickets per item)
In addition to the sale of tickets we will be offering two types of game pass; a "Play Every Game" card that will allow one participant to play each game one time and an "Unlimited Games" wrist band that will allow one participant to play as many games as they want throughout the 2 hr event. When making your purchases please be advised that tickets will still be needed for activities and food, even if a game pass is purchased to play games. 
$1 tickets - used for games, activities, and food
Card allows participants to play each of the 11 games 1 time
*for use on games ONLY, activities not included
All purchases made using this form may be picked up from the office during the week by contacting the administrator or on the day of the carnival, in the main office.
